Group/Centre Activities
We highly encourage you to engage in independent or group activities to assist you in exploring your passion and hidden skills. By participating in these activities, you will socialize with our support workers as well as connect with other participants in our community. Some of our group and centred based activities are arts and crafts, kitchen and cooking skills classes, music, game and more.
Support Coordination
Through support coordination, we will assist you by coordinating the services you need from your NDIS plan, connect you with specialised care and community networks to reach your goals and other NDIS information that you are not familiar with.
Specialised Support Coordination
Our Specialised Support Coordination is a higher level of support coordination for participants with complex situations who need specialist support. Our specialist support coordinator will assist and manage your challenges, learn your needs and environment, develop a support plan, and ensure that they are consistently delivered to you.
Therapeutic Supports (Allied Health)
We understand, recognise, and respect all our participants’ needs in achieving their specific goals. Hence, we work with other health and community support groups including your family and friends to provide you with strong network support. Some of the therapeutic support offered by our highly experienced and qualified therapist are general counselling support, depression and anxiety counselling, personal development, and confidence building strategies.
Plan Management
We ensure that your NDIS fund is professionally managed by our highly skilled team by assisting you in your payment, claims and the administration of your NDIS budget.
Community Participation
Need some assistance in getting out of home, into the community to build your skills and feel included with your disability? We will assist and engage our participants with recreational activities and classes that support their needs and goals.
Interpret / Translate
Our professional team of translators and interpreters are well versed with many languages, hence we shall assist our participants with their preferred language to channel clear communication.
Household Tasks
A healthy environment at home creates a healthy and happier you at home. To ensure that you feel healthier and happier, Donum Dei will assist you with your home care such as your home maintenance, spring cleaning, nutritious meal preparation, laundry, and other household tasks.
Early Childhood Supports
Donum Dei’s team will assist families with children aged 0-6 with a disability by providing families with assistance, advice, and access to the support of your local community. Besides, our experts will provide early intervention support for your children such as childcare, sports and community activities, and playgroup to reduce their possible need for long term intervention.
Development-Life Skills
To make the most out of your life independently, life skills are essential to help you work towards your goals and live your life to your fullest potential. Our team will work with you to build the essential skills you need in life like communication and social skills, problem solving skills, manage personal finances, literacy and numeracy skills, meal planning and preparation and many more.
Innovative Community Participation
Innovative Community Participation is similar to community participation; however, this is specific to participants who are interested to be involved in support offered by Donum Dei. You will be learning new skills, building existing skills, socialising, and expanding your network of friends.
Daily Tasks/Shared Living (Supported Independent Living – SIL)
This service is to support you to live independently as possible. We will support you at home or out in the community. Our services varied depending on your requirements to achieve your personal goals such as budgeting, shopping, completing forms, paying bills and maintaining your health and wellbeing.
Community Nursing Care
Community Nursing Care is provided to participants who need complex support. Hence, our professional nurses who are well trained and experienced shall meet the health needs of the participants. Our complex support includes complex bowel care, urinary catheter management, complex wound management and more.
Assist-Travel/Transport
Travel or transport assistance allows you to complete your daily tasks and to live a sustainable life. At Donum Dei, our team shall assist you through your travel plan and transport to fulfil your important tasks. Our support includes assisting you to use public transport, visiting your appointments, community events and more.
Assist-Personal Activities
Aging or recovering at home is the best environment to be comfortable in. However, handling personal activities at home alone is inconvenient. Hence, our team of experts assist our participants with their personal care such as everyday dressing and grooming, feeding or assistance with eating, bathroom breaks, medicine tracking and attending events.
Assist-Life Stage, Transition
Transitioning to a new stage of life is challenging and it can be much tougher if you have a disability. Our professional team will help you through all life stages transition from attending school, leaving home, getting your first job and retirement. Our life transition support includes mentoring peer support, individual skill development, and support connection and coordination.
Assist Access / Maintain Employ
Apart from preparing for a job interview, we will assist you at exploring and learning new skills, gaining confidence and landing meaning employment opportunities that are tailored to your skills and abilities.
Accommodation / Tenancy
To steer your life towards independence and achieving your daily goals, we will assist you at undertaking activities to ensure that you obtain appropriate accommodation. This includes applying for rental tenancy or to undertake tenancy obligations. Our service also includes case planning, budgeting, and liaising with service providers for home modifications to assist your disability.
